Fight It

A Support & Assistance Program from Diabetic Rockstar

Request Assistance

Diabetic Rockstar’s Fight It! Campaign does not discriminate against or screen our applicants based on any factor such as age, race, creed, gender, sexuality, nationality, and language. Demographic and personal information such as employment status, yearly income and medical insurance coverage may be used to determine the value of an assistance package or aid award, or the approval/denial itself the application itself.

Presently the Fight It! Campaign is approving nearly 60% of applications for some sort of assistance. Value of these awards averages around $250 (based on wholesale values of supplies), and can include a variety of aid types, as detailed below. Each Fight It! Campaign applicant is individually and independently reviewed, insuring that previous applicants’ awards/decisions and prior circumstances do not affect the current applicant. This additionally insures the applicant a fair, unbiased and bipartisan review and decision.

Types of Aid/Assistance

Diabetes Management Supplies

A high percentage of assistance/aid awards are completely management-supply based, or at least partially comprised of. These supplies include blood glucose testing meters, testing strips, lancets, pump supplies (ie infusion sets), glucose tablets, and, depending on applicable municipal laws, insulin, syringes and pen needles.

Advanced Care-Technology

In special circumstances we will award assistance packages that include new technology; examples include at-home Hemoglobin A1C tests and CGMS supplies/parts/units.

General Medical Care Reimbursement

In unique circumstances, and when financial conditions allow, we may award monetary reimbursement for general medical care or offer an advance payment solution for this care. This includes general patient check-ups, eye exams, a1c/bloodwork, etc. Assistance/Awards of this type are rare and usually reserved for special cases.

Advanced Medical Care/Emergency Care Reimbursement

Similar to the previous “general” care, we may award monetary reimbursement/payment for advanced medical or emergency care. Examples of advanced medical care are diabetes-related surgical procedures and ketones-related hospitalizations, along with other in-patient services. Emergency care must be solely due to diabetes complications, such as a severe hypoglycemic episode. More than “General Medical Care Reimbursement”, these assistance awards are extremely rare and most likely would necessitate in-person consultation with the applicant and medical/billing documentations.

Mental Health

One of Diabetic Rockstar Inc’s missions is to raise awareness of mental health as it relates to diabetes. This includes cases of depression and eating disorders, such as diabulemia, and several others. Advanced consultation between the Fight It! Campaign advisors/directors and the applicant must occur prior to any assistance package of this type being awarded; if so, aid such as medical/pharmaceutical reimbursement and therapy sponsorships may be awarded.

Purchasing of Unique Management Supply Items/Bulk Purchasing

In many cases applicants may require unique diabetes management supplies, an inordinate number of a certain type of supply, or the state/municipal laws do not allow for the Fight It! Campaign to facilitate proper and mutually satisfactory aid awards. Diabetic Rockstar Inc and the Fight It! Campaign have put in place a system that allows for resolution to these issues, and will work privately with the applicant case-by-case.

Coupons/Discounts/Promotions

One of the major benefits of having a highly visible network (www.diabeticrockstar.com), and charitable vision with Fight It! that many people can relate to, is that we have forged partnerships with some great companies. Additionally we receive a number of promotional items and great resources for savings on diabetes supplies, or even free supplies, as well as care programs developed by other companies that may have a larger operating budget than us. In every way possible Diabetic Rockstar and Fight It! will directly pass these on to applicants.

As earlier stated, these are some of the more likely assistance/aid types the program will award. They are not a complete list, however. Each application is an individual, with a unique background and situation, and Fight It! handles it like so.

To Apply

Please download the application form below. The application is ten pages, and should be completed in its entirety. Please keep in mind the following while completing your applications:
  1. Please write legibly. Applications that cannot be read by the reviewer won’t be processed. This does not mean your application will be denied, but you will lose your position as we process “properly completed” applications in the order we receive them.
  2. Be thorough and detailed. Too much information is far better than too little. Detailed information allows us, if granted, to create an award package uniquely tailored to your diabetes management needs and situation.
  3. Attach additional paper if necessary. Please do not feel limited by the spaces provided. Attach additional paper for your answers if needed, or even provide a type written copy (if you choose to complete your application via Word or similar please make sure that your answers line up correctly with the original application).
  4. Provide necessary information or supporting documentation. We do ask for info such as employer name and phone number, or insurance carrier. These types of questions are to insure that we are following all state and federal guidelines/laws/restrictions, as well as insuring that all applicants have a fair review.
  5. Sign and Date… and double check. Applications not completed filled out will not be processed, nor will apps that are not signed and dated.
  6. Parent/Guardian financial information is needed if the diabetic patient/applicant is a minor.
